Thank you for your interest in our Faith Formation programs!  We use the same form for all of our youth programs.  Please fill out one form for your entire family.

Early Bird: February 12 - April 30
Regular Fee: May 1 - June 30
Late Fee: July 1 - July 31
Registration closes August 18.
