  Baptisms must be verified. IF THE CHILD’S BAPTISM DID NOT TAKE PLACE AT ST. MARY OF THE EXPECTATION, request a certificate “with notations” from the church of baptism. It must be dated within six months of this application and showing the church’s seal. In the State of Texas, a baptismal certificate is a legal document and therefore must be accurate. Verify that all information on the certificate is correct. If changes are required, contact the church of baptism and ask for an amended certificate. Submit certificates to the proper office (see contact information below).
  
  LEGAL GUARDIANSHIP:
  Both birth parents must be aware of the sacramental preparation of this child, and be at least neutral in it. If divorced or legally separated with religious rights for the child, please provide copies of the original court documents granting this right, both the judge’s signature and date of decree. If no specific right has been granted, then provide a brief letter from the non-custodial parent stating he/she is aware of (not necessarily approving of) the preparation of this child.
